• This is suitable for participants with more defined and high support needs. 
• Your Specialist Support Coordinator will help to coordinate consistent service delivery and carry out health outcomes. 
• This is provided by a professional with an allied health background or psychological experiences such as a Social Worker or Occupational Therapist. 

Specialist Support Coordinators will provide all services planned above, but also may provide specialist support coordination services depending on a participant’s scenario and higher levels of taking a risk. Participants who face additional barriers and have matters that are time-sensitive would benefit from these services provided by the platform.

• Specialist Support Coordination is support coordination that is provided by a professional with Psychological expertise. 
• This is funded for complex participants who have exceptionally different needs or are in a crisis situation.
• Often when Specialist Support Coordination is funded where the participant may face additional and high-risk barriers to support.
